1. Review Provider Contract: Familiarize yourself with our provider contract to ensure you understand the terms and expectations.

2. Schedule Sessions: When a client contacts you, confirm they are on a REPS referral and schedule their sessions.

3. Provide Counseling: Conduct the five complimentary and confidential counseling sessions.

4. Complete and Submit Provider Form: After the final session, fill out the form below with session details and submit it for reimbursement.
