2025 Yampa Valley Community Health Needs Assessment
Northwest Colorado Health, UCHealth, Memorial Regional Health, The Health Partnership, and the Public Health departments of Routt and Moffat counties have collaborated to launch the CHNA to identify the most critical health issues facing our communities. A key component of this process is a community-wide survey that is available online now until February 21, 2025. The survey is available in both English and Spanish.
Moffat and Routt County community members 18 years and older are encouraged to participate. Survey responses are anonymous and it takes approximately 13 minutes to complete.

Yampa Valley Establishments Pledge To Be "ALL IN"
In response to the recent tragic loss of a beloved member of our local service industry to suicide, we're launching the "All In" Campaign. This initiative aims to address mental health concerns within our community, particularly in the service industry. With the Rocky Mountain Region's alarming suicide rates and the Western Slope's disproportionately high numbers, it's clear that action is urgently needed. By pledging to be "All In," local establishments are committing to advocate for a suicide safer community, both within their business and throughout the Yampa Valley. Together, let's change the conversation around mental health and support one another.
If you are interested in getting involved in this campaign either as a volunteer or with your local establishment, click the button below for more information.
